Please Read Before Your Visit
  • Do not delay—seek medical care as early as possible. Early evaluation is a key step toward a healthy pregnancy, and younger age significantly improves success rates.
  • Choose a specialized fertility clinic. A general OB-GYN exam may not identify the exact cause of infertility. Accurate diagnosis and treatment require experienced specialists and advanced facilities.
  • Trust your doctor and follow the treatment plan. Every case is unique—avoid comparisons and focus on your own cycle and condition.
  • Fertility testing should be done as a couple. Pregnancy involves both partners. Joint evaluation is essential.
  • Fertility tests may be uncomfortable, but they are not painful. Please feel at ease during the process.
  • Prepare your medical history before your visit. You will be asked detailed questions about your health and pregnancy history. Preparing in advance will help ensure an accurate consultation.
1For Both Partners
  • Have you used contraception after marriage?
  • How long have you been trying to conceive?
  • How often do you have intercourse? (Frequency & duration)
  • Any issues with sexual activity or pain during intercourse?
  • Are you currently taking any medications?
  • Have you had any surgeries or hospitalizations in the past?
  • Have you received fertility evaluation or treatment at another clinic?
  • If yes, what tests or treatments did you undergo?
2For Women
  • At what age did your first period begin?
  • Menstrual history (cycle, duration, flow, pain)
  • Any previous pregnancies? (Delivery / Induced abortion / Miscarriage)
  • If you experienced miscarriage, were there any changes in your cycle afterward?
3For Men
  • Have you ever had a high fever illness during growth? (e.g., mumps, measles)
